P2408 means the ECM is seeing an out-of-range or open circuit on the fuel-cap sensor/switch input — on platforms with a tethered cap and an integrated cap-presence switch, or on capless filler systems (Ford EasyFuel, Toyota Prius capless, late-model GM) where a flapper-position microswitch reports whether the fuel door's internal valve is seated. Typical electrical signature: 0V on the signal pin with the cap installed/flapper seated, 5V with the cap off/flapper open (some platforms invert this — confirm in the FSM before chasing voltage). The code sets when the signal stays at one rail regardless of physical cap state, indicating a broken signal wire, a stuck microswitch, or a failed cap-presence sensor. Cheapest-first ladder: pop the fuel door and physically inspect the cap or capless flapper for damage, then back-probe the signal pin at the sensor and confirm the voltage transitions correctly when you actuate the flapper or remove/install the cap, then continuity-check the signal wire from the sensor connector to the ECM/BCM. Expensive misdiagnosis caveat: capless fillers like Ford EasyFuel have a flapper-position sensor that often reads as cap-off after a fuel fill because residual fuel or debris around the flapper rim prevents a full seal — a drive cycle and a clean flapper should clear the code before condemning the assembly; do not replace the whole capless filler neck (a $300-500 part) until you've cleaned the flapper seat and verified the code returns.

Vehicle-specific patterns: 2010+ Ford EasyFuel capless (Fusion, Fiesta, Escape, Edge, Focus, F-150) is the most common P2408 ticket in modern shops — debris around the flapper microswitch, a bent flapper from a misused funnel, or a stuck switch all set the code; clean the flapper seat and check operation before replacing the filler-neck assembly. 2008-2015 Hyundai/Kia (Sonata, Optima, Elantra, Forte) with a tethered cap and integrated cap-presence sensor throw P2408 when the sensor wire chafes against the filler-neck shield. 2010+ Toyota Prius capless throws P2408 from a flapper-microswitch failure that frequently corrodes from condensation inside the filler housing. Estimated repair: $50 to $450.
