P0254 means the injection pump fuel metering control 'A' circuit signal is pinned high — the ECM is reading near rail voltage (12V or 5V depending on platform) on the SCV/FCA feedback path, which typically indicates a short-to-power, an open-circuit on the ground/return leg, or a solenoid driver fault in the ECM. As with P0253 this is almost always an electrical fault on the metering solenoid, not a mechanical pump problem. Cheapest-first diagnostic ladder: (1) Unplug the SCV/FCA connector and inspect for fuel saturation, melted insulation (heat damage from exhaust or turbo proximity), and signs of a previous repair attempt — many P0254 vans/trucks have a 'mystery splice' that's broken or backwards. (2) Ohm the SCV coil — Bosch CP3 SCV reads 1.5-3 ohm, Bosch CP4 SCV reads 2-5 ohm; an open reading confirms a broken internal winding which would cause the driver to see rail voltage on the feedback line. (3) Back-probe both sides of the connector key-on and look for one wire stuck at battery voltage when commanded; a wire that should be PWM-switching but reads steady 12V is either open downstream (broken wire between solenoid and ECM) or shorted to power. (4) Verify the ECM ground at the solenoid return — corroded ECM ground straps can cause the driver to read high voltage even with a good solenoid. (5) Inspect for melted harness insulation along any turbo/EGR/manifold heat-exposure path; chafed insulation that shorts the SCV wire to a B+ feeder is a not-uncommon P0254 source on platforms with cramped engine bays. Live-data the SCV duty cycle: it will read pinned at 0% or 100% with the rail-pressure feedback far from target. Expensive-misdiagnosis caveat: P0254 paired with low rail pressure can look like a failing CP3/CP4 pump, but the code itself is electrical — confirm the solenoid is bad and the harness is intact before quoting a pump. A $200 SCV swap clears most P0254s where a pump replacement would be malpractice.

Vehicle-specific patterns: 2001-2010 GM Duramax LB7/LLY/LBZ/LMM throws P0254 from heat-damaged FCA harness near the up-pipe / Y-pipe — the loom routes uncomfortably close to exhaust on the back of the engine and the insulation cooks at high duty cycles. 2003-2018 RAM Cummins 5.9L/6.7L throws P0254 from the same fuel-saturation pattern as P0253 plus heat damage near the turbo pedestal. 2011-2016 Ford 6.7L Powerstroke throws P0254 occasionally from harness damage in the lifter valley around the CP4 — service that requires removing the intake or EGR cooler often disturbs the loom. 2008-2010 Ford 6.4L Powerstroke throws P0254 from heat damage to the CP3.3 SCV harness near the EGR cooler and turbo. 2009-2014 VW TDI 2.0L CR throws P0254 from CP4 SCV winding open-circuit failures. 2007-2018 Mercedes Sprinter OM642 throws P0254 from quantity control valve harness damage where the loom passes the turbocharger heat shield. Estimated repair: $50 to $600.
