P0190 is a generic-circuit fault on the high-pressure fuel rail pressure (FRP) sensor — the sensor mounted directly on the fuel rail of a direct-injection or common-rail diesel engine that reports actual rail pressure back to the ECU in real time (typical operating ranges: 500-2,200 psi on gas direct-injection, 5,000-30,000 psi on modern common-rail diesels). When P0190 sets, the ECU is seeing a signal it can't reconcile and almost always defaults to limp mode — hard-start, no-start, or severely reduced power — because injection timing and duration depend on knowing rail pressure to within a few percent. Diagnostic order on a shop floor: (1) Scan-tool the desired-vs-actual fuel-rail-pressure PIDs at KOEO and at crank — if actual reads zero or pegged max while desired tracks normally, the sensor or its wiring is the suspect; if both desired and actual track together but the ECU still sets P0190, the fault is intermittent. (2) Inspect the sensor connector — fuel-rail sensors live in a hot, vibration-heavy environment and the connector seal is often the first thing to fail; pull it, look for corrosion or oil/fuel seepage, and reseat with dielectric grease. (3) DMM the 5V reference, ground, and signal wires at the connector with KOEO; a missing 5V reference points to an ECU-side problem and a missing ground points to a harness fault. (4) Only after wiring is verified, swap the sensor — they fail internally but it's not the #1 cause. Critical safety note: do NOT crack the sensor port on a common-rail diesel with the engine running or recently shut down — rail pressure can persist long enough to inject diesel through skin under skin-penetration pressure.

Vehicle-specific patterns: 2003-2007 Ford 6.0L Power Stroke is the classic P0190 vehicle — the high-pressure oil rail sensor (IPR-adjacent on this engine) and the fuel rail pressure sensor are both common failures and Ford issued multiple TSBs; expect $200-$400 installed on a known-good aftermarket sensor like Motorcraft. 2008-2010 Ford 6.4L Power Stroke and 2011-2019 6.7L Power Stroke also throw P0190 routinely from the rail-pressure sensor itself, often paired with hard-start complaints. 2004-2010 GM Duramax 6.6L LLY/LBZ/LMM throws P0190 from a fuel-rail pressure sensor failure on the driver-side rail — $150-$250 sensor, accessible with normal hand tools. 2010-2016 BMW N55 / N20 / N63 turbo direct-injection gas engines throw P0190 from HPFP-adjacent rail sensor failures or from a failing high-pressure pump that can't hit commanded rail pressure (the sensor correctly reports the low value, ECU blames the sensor) — verify pump output before parts-throwing. 2013-2018 RAM Cummins 6.7L and 2007-2018 Dodge Cummins 6.7L throw P0190 from the rail sensor or from contamination after a CP4-style pump failure that sends metal through the rail. Repair cost typically $150-$700: sensor + connector at the low end; if the underlying cause is a failed high-pressure pump (especially the dreaded CP4 on Power Stroke 6.7L or VW/Audi TDI applications), the real bill climbs to $4,000-$12,000 with full fuel-system contamination cleanup.
