P0149 is a fuel timing error — primarily a diesel and direct-injection code where the ECM detected that actual injection timing (start of injection, or SOI) deviates from the commanded timing by more than the threshold (typically 2-5 crankshaft degrees for 5-10 seconds). On modern common-rail diesels, this code reflects fuel-pressure-regulator response lag, injector solenoid response time drift, or camshaft-position-sensor offset error feeding bad timing data to the injector driver. On GDI gasoline engines, P0149 typically reflects high-pressure-pump phasing error against the camshaft drive lobe. Cheapest-first ladder: (1) Pull freeze-frame data and check engine speed, load, and rail pressure at the moment of the fault — most P0149 events occur at high load with rail pressure under 18,000 psi (diesel) or 1,800 psi (GDI gas), pointing back to a pressure-supply issue rather than a timing issue. (2) Scan camshaft position sensor signal correlation against crankshaft position — a worn or stretched timing chain creates a 1-3 degree offset that masquerades as a timing fault. (3) Check injector response data — some scan tools display individual injector ON-time and balance; a slow-responding injector (solenoid coil drift or worn seat) triggers SOI deviation. (4) Inspect the high-pressure pump drive coupling for wear; on Ford EcoBoost and Audi/VW TSI engines the cam follower bucket wears through and changes pump phasing. The expensive misdiagnosis is condemning the timing chain ($2,500-$5,000) when the actual fault was a $400 cam follower or a single injector at $500.

Estimated repair: $350 to $6,500.
