P1349 is a Toyota VVT-i system malfunction set when the ECM commands the intake cam to advance or retard and the actual cam position (read from the camshaft position sensor) does not match the commanded position within a calibrated window, typically more than 5 to 10 crank degrees of error sustained for several seconds. The system uses an Oil Control Valve (OCV) that meters engine oil to the cam phaser; when the OCV screen plugs with sludge or the phaser locking pin fails, the cam either won't move or moves erratically. Cheapest-first ladder: change the oil and filter first if it's overdue, then remove the OCV (one bolt on the front of the head on 1MZ/2AZ/2GR), inspect the inline screen for varnish and metal flake, and clean with brake cleaner. Bench-test the OCV with 12V across the terminals; the spool should snap audibly and freely. Live data: monitor VVT Target Angle vs VVT Actual Angle in Techstream during a 1500 rpm hold; deviation under 3 degrees is healthy, sustained deviation over 5 degrees confirms the fault. The expensive misdiagnosis is replacing the cam phaser ($400 to $900 in parts plus 6 to 10 hours labor) when the real fault was a $12 OCV screen clogged from a missed oil change, or a $90 OCV solenoid that bench-tests bad.

Toyota 2AZ-FE 2.4L (Camry, RAV4, Solara, Scion tC 2002-2009) is notorious for OCV screen clog driven by the engine's well-known oil consumption problem. Toyota 2GR-FE 3.5L V6 (Camry, Avalon, Sienna, Venza 2007-2014) typically throws P1349 from a stuck OCV solenoid rather than screen clog. Toyota 2ZR-FE 1.8L (Corolla, Matrix 2009-2018) sets P1349 most often from a worn intake cam phaser internal locking mechanism that rattles on cold start. Estimated repair: $40 to $1,200.
