P0AFA indicates the hybrid battery system voltage has dropped below the BECM's low-side operational threshold — on a Toyota Prius Gen 2 NHW20 pack (nominal 201.6V), this trips below approximately 168V under load or 180V at rest; on Chevrolet Volt (nominal 355V usable, 360V nominal lithium) the trip is around 280V. The BECM disables propulsion when this code sets because operating at low pack voltage damages cells through over-discharge. Cheapest-first diagnostic ladder: confirm the pack is not simply deeply discharged from extended sit — a hybrid that has sat for 6+ weeks without driving will self-discharge enough to throw P0AFA on first wake. Pull block voltages and look for one block dramatically below the others — a single shorted cell in one block can drag the whole pack's terminal voltage below threshold even though 27 of 28 modules are healthy. Caveat: P0AFA is often the first code thrown when a Prius pack starts dying — it precedes P0A7F and P0A80 by weeks or months in many cases.
