P0AF3 sets when the Battery Energy Control Module (BECM/BMS) sees the voltage-sense "C" circuit reporting a value that's plausible electrically but inconsistent with the pack's expected behavior — typically a sub-pack or module-group voltage that drifts away from its neighbors during charge/discharge cycles. The cheapest-first ladder: pull freeze-frame and look at per-module voltages on the scan tool (Techstream for Toyota, GDS2 for GM, ISTA for BMW). A healthy Li-ion module group sits at 3.6-4.2V per cell; a NiMH stick (Prius Gen2/Gen3) reads 7.2V nominal per stick (1.2V per cell x 6). If one sense input lags its neighbors by more than 0.2V at the same SOC, that's the suspect group. Inspect the BMS sense harness at the module bus-bar bolts — fretting corrosion and chafing at the bolt head is the #1 cause and presents as an intermittent range/performance fault, not a hard open. Confirm pack-level voltage at the service plug with the isolation tester: 200-650V scaled depending on architecture (Prius Gen2 ~202V nominal, Volt Gen1 ~360V, Bolt EV ~360V, Tesla packs 350-400V). DO NOT probe HV orange-wire circuits with a standard multimeter. Always service HV with Class 0 1000V gloves, isolation tester (Megger), and DC-rated lugged probes. Voltage-sense codes (P0AF3-P0AF7) often indicate the BMS sense lead chafed at the bus-bar bolt — re-tightening with new lockwasher can fix without pack replacement.

Vehicle-specific patterns: 2004-2009 Toyota Prius Gen2 (NHW20) throws P0AF3 from BMS sense-lead failure at 100-180K mi — the orange ring terminals at the NiMH module bus-bar bolts fret against the copper bus-bar from years of pack thermal cycling, and contact resistance climbs until the sense voltage drifts. Re-tightening with a fresh stainless lockwasher and a smear of NyoGel 760G dielectric grease often restores the circuit without touching the pack. 2010-2015 Toyota Prius Gen3 (ZVW30) shows the same pattern but compounded by tabs cracking on the BMS sense ribbon. 2011-2015 Chevy Volt Gen1 throws P0AF3 from the LBC (Lithium Battery Controller) voltage-sense harness corroding at the through-pack connector — GM TSBs cover several voltage-sense pin issues but verify against current bulletins, don't quote numbers from memory. 2017-2019 Chevy Bolt EV with the LG cell module pack throws P0AF3 as an early warning sign of the cell-module imbalance issue that GM addressed via the well-known Bolt battery recall — check the VIN against current recall coverage before quoting any pack-level work. Estimated repair: $180 (sense-harness re-terminate + lockwasher kit) to $4,500 (module-group replacement) to $15,000+ (full pack on a Bolt outside recall coverage).
