P0A09 is set when the DC-DC converter (the inverter sub-assembly that steps the HV pack voltage down to the 12V bus to charge the auxiliary battery and run all conventional 12V loads) reports a status signal voltage below the expected operating window on its dedicated low-voltage feedback line to the HV ECU. The cheapest-first ladder starts with the 12V auxiliary battery itself: P0A09 is famous for being thrown when the aux 12V battery is dying or has a corroded terminal, because the DC-DC converter sees abnormal load characteristics and reports an out-of-range status. Load-test the 12V auxiliary battery (Prius and most Toyota hybrids use a small AGM that should hold 12.4-12.7V rested and 9.6V minimum under 100A crank load; below spec means replace before chasing the converter). Confirm DC-DC converter output at the aux battery with a DMM: with READY mode on, the bus should read 13.8 to 14.4V (some platforms run as low as 13.5V at full pack SOC and as high as 14.6V during regen). Below 13.0V means the converter is undervolting or not running. Above 14.8V means runaway - pull immediately. Inspect the converter status signal connector (typically a low-voltage 2-3 wire pigtail separate from the orange HV cables) for pin push-out and corrosion. DO NOT probe HV orange-wire circuits with a standard multimeter. Always service HV with Class 0 1000V gloves, isolation tester (Megger), and DC-rated lugged probes. The HV interlock loop drops pack contactors when the service plug is pulled; never bypass at lethal risk.

Vehicle-specific patterns: 2004-2009 Toyota Prius Gen 2 throws P0A09 most often from a tired aux 12V AGM battery; replace the AGM, clear the code, and the converter is fine in 60-70% of cases. 2010-2015 Prius Gen 3 and 2012-2015 Prius Plug-In share the same converter architecture and the same aux battery sensitivity. 2007-2017 Camry Hybrid and Highlander Hybrid throw the code from the same root cause. 2011-2019 GM Volt Gen 1/Gen 2 throws P0A09 when the Voltec drive unit isolation degrades and the DC-DC converter status feedback drifts; GM service info covers the isolation diagnostic procedure with a megohmmeter. 2003-2015 Honda Civic Hybrid (IMA) throws a similar status code when the 12V converter integrated into the IPU sees aux battery dropout; Honda issued a large warranty extension on the IMA battery pack that often resolves these adjacent faults. Estimated repair: $180 (aux 12V battery) to $2,800 (converter replacement).
