P0281 is the cylinder 7 contribution/balance fault — only valid on engines with 7 or more cylinders, so in shop work this means a V8, V10, V12, or an inline-8. The ECM has determined that cylinder 7 deviates from the average per-cylinder fuel trim by more than +/- 5 mm3/stroke for a sustained period. Cheapest-first ladder: scan live cylinder balance trims (normal 0-3 mm3 deviation, flagged at greater than 5 mm3), run a cylinder cutout test with cyl 7's injector disabled (healthy drop 80-150 RPM, weak drop less than 50 RPM), then compression test cyl 7 (looking for less than 85% of the highest-reading cylinder, ideally within 10% of the average). Then injector electrical check (resistance per OEM spec, typically 0.5-2.0 ohms on solenoid injectors, 1.5-3.5 ohms on piezo) and a back-leak / return-flow test on common-rail diesels. On gasoline direct-injection engines, also pull cylinder 7's intake valve via borescope to check for carbon buildup that can throw a contribution code without setting a misfire. Expensive-misdiagnosis caveat: on a V8 with a worn timing chain or stretched cam phaser, valve timing on one bank drifts and one cylinder's volumetric efficiency collapses — replacing the injector won't fix this and you'll be back into the engine for $2,000-$4,500 in chain/phaser work. Always verify cam timing correlation (typically less than 5 degrees crank deviation between banks at hot idle) before condemning injectors.

Vehicle-specific patterns

Vehicle-specific patterns: 2008-2010 Ford 6.4L Powerstroke V8 throws P0281 from the same piston cracking and high-pressure-pump contamination issues that hit P0278 — cylinder 7 is on the left bank and frequently shows up paired with P0275 and P0278 because all left-bank injectors degrade together. 2007.5-2010 Duramax LMM and 2011-2016 Duramax LML 6.6L V8 commonly throw P0281 when an individual injector starts cracking internally and bleeds rail pressure; cyl 7 is on the left bank rear. 2011-2018 RAM 2500/3500 with the 6.7L Cummins inline-6 cannot throw P0281 (only 6 cylinders) — if you see it, suspect a generic-code-table misread or an ECM flash issue. 2008-2014 BMW N63 4.4L twin-turbo V8 throws P0281 from the same coil/injector failures and timing-chain stretch that cause widespread misfire and balance codes; BMW addressed the N63 with a Customer Care Package extending warranty on injectors and coils. 2010-2017 Audi 4.2L V8 FSI (RS5, S5) throws cylinder-7 balance codes from carbon buildup on the rear-bank intake valves. Estimated repair: $200 (intake valve walnut blast) to $6,000 (full injector set + cam phasers on a 6.4L Powerstroke).
