P0169 means the ECM has detected that the actual fuel composition doesn't match what the flex-fuel composition sensor (or the inferred ethanol-percentage calculation from fuel trim plus crank-speed analysis) is reporting — typically on FFV gasoline platforms that need to know ethanol percentage to set the correct injector pulse-width, since E85 requires roughly 30-40% more fuel by volume than straight gasoline to hit the same lambda. Flex-fuel sensors read ethanol percentage 0-85% (E0 through E85) and feed it to the ECM as a frequency-modulated signal (typically 50-150Hz, where the frequency maps linearly to ethanol percentage). When P0169 sets, the fuel-trim correction needed to hit closed-loop lambda is wildly inconsistent with the reported ethanol percentage — for example, the sensor says E10 but the ECM is pulling -25% short-term fuel trim to keep lambda at 1.0, which only makes sense if the fuel is actually E60+. Cheapest-first ladder: scan ethanol-percentage live data, then drain a sample from the tank into a clear container and add water (gasoline floats, ethanol mixes with the water — the volume increase tells you actual ethanol percentage) and compare against the scan tool reading; verify STFT/LTFT on both banks (normal +/-10%, pegged +/-25% means closed-loop is fighting a fuel-composition mismatch). If the scan tool says E10 and the water-shake test confirms E10 but trims are still pegged, the sensor's frequency output is drifting or the ECM is misinterpreting it. Expensive-misdiagnosis caveat: a leaking injector or a stuck-open EVAP purge valve can dump enough fuel into the manifold to mimic a high-ethanol fuel-composition error — verify those before ordering a $400 flex-fuel sensor.

Vehicle-specific patterns

Vehicle-specific patterns: 2006-2012 GM Impala/Tahoe/Suburban FFV with the 5.3L/3.5L flex-fuel platforms commonly throws P0169 from the in-line ethanol composition sensor (mounted in the fuel feed line near the rail) failing internally after 80-120k miles. 2003-2010 Ford F-150/Explorer FFV with the 4.6L/5.4L throws P0169 from harness corrosion at the composition-sensor connector under the truck. 2007-2012 Chrysler Sebring/200/Avenger FFV with the 2.7L/3.5L throws P0169 from the sensor element drifting after long-term E85 duty. 2009-2014 RAM 1500 FFV with the 5.7L Hemi throws P0169 from a known sensor recall — check VIN for the redesigned-sensor coverage.
