P0166 means the bank 2 sensor 3 O2 sensor shows no activity at all — the signal voltage is stuck at the bias reference (typically 0.45V) and never deviates regardless of commanded fuel changes, throttle position, or engine load. This is electrically distinct from P0162 (out-of-range circuit fault) and P0165 (slow response); P0166 specifically means the sensor element is alive enough to maintain bias but isn't generating any signal. The two most common causes are a cold sensor (heater circuit failed, sensor never reaches 600F+ light-off temperature) and a contaminated sensor element (silicone glazing, lead poisoning, oil ash buildup blocking oxygen diffusion to the platinum element). Cheapest-first ladder: scan heater current at cold start — a healthy narrow-band heater pulls 0.5-2A for the first 30-60 seconds before tapering as the sensor self-heats; zero heater current means the heater is open-circuit and the sensor is freezing cold even though the exhaust is hot enough. Ohm the heater element cold (typical 6-15 ohms) and confirm the heater B+ feed is hitting 12V with key on. Verify the sensor ground returns to chassis under 0.1V drop. If the heater checks out, pull the sensor and inspect the tip — a clean tan/gray tip with no deposits means the sensor is fine and you're chasing a wiring or ECM issue; a white-glazed or oil-fouled tip means the sensor is dead from contamination. Expensive-misdiagnosis caveat: an ECM with a failed O2 driver circuit can produce P0166 identically to a dead sensor — verify the bias voltage actually pulses on commanded changes by back-probing at the ECM connector before condemning the sensor.

Vehicle-specific patterns: 2003-2009 Toyota 4Runner/Sequoia 4.7L 2UZ-FE throws P0166 commonly from the heater driver circuit inside the ECM failing — Toyota issued a TSB for ECM reflash on affected VINs before condemning the sensor. 2005-2010 Ford Expedition/Navigator 5.4L 3V throws P0166 from connector pin-fretting at the bank 2 sensor 3 plug; the pins look fine but the contact resistance climbs and starves the heater. 2008-2014 RAM 2500/3500 6.7L Cummins (which uses post-cat O2 for the DPF/SCR strategy) throws P0166 from soot accumulation glazing the sensor element. 2010-2016 Range Rover/Range Rover Sport 5.0L V8 NA and supercharged throws P0166 from coolant intrusion through the valley-pan-to-head gasket onto the post-cat sensor wiring. Estimated repair: $150 to $650.
