P0165 means the bank 2 sensor 3 O2 signal is responding too slowly to commanded fuel changes — the ECM commands a rich or lean transition via injector pulse-width and times how long the post-cat sensor takes to cross the 0.45V threshold (or for a wideband, how long lambda takes to move 0.1 either side of 1.0). Slow response on a post-cat sensor is the most common precursor to a catalyst-efficiency code (P0430 family) because the cat itself is buffering the response, so the sensor reads slow even though it's mechanically fine. Cheapest-first ladder: scan O2 live data with the engine warm at 2,500 RPM and command a forced rich/lean transition via the scan tool's bidirectional control if available; a healthy narrow-band sensor should cross 0.45V within 100-300 ms of the commanded change. A sensor that takes 800+ ms or fails to fully swing 0.1-0.9V (or 0.7-1.3 lambda on a wideband) is either contaminated (silicone, lead, sulfur, oil ash) or aged out. Pull the sensor and inspect for white powder (silicone from RTV gasket maker), green crust (coolant), or black sooty deposits (oil burning). Verify the heater pulls 0.5-2A — a weak heater means the sensor never reaches its 600F+ operating temperature and reads slow because it's cold, not because it's bad. Expensive-misdiagnosis caveat: a slow post-cat sensor reading with a healthy upstream sensor on the same bank usually means the cat itself is dying and the sensor is honestly reporting low oxygen storage — don't replace the sensor if the upstream is swinging full range and the downstream is barely moving; that's a P0430-bound cat, not a bad sensor.

Vehicle-specific patterns: 2007-2014 Toyota Tundra/Sequoia 5.7L 3UR-FE throws P0165 commonly as a precursor to P0430 — the post-cat sensor isn't bad, the cat's oxygen storage capacity has dropped below threshold and the sensor honestly reflects it. 2009-2015 RAM 1500/2500 with the 5.7L Hemi MDS throws P0165 from oil consumption past the MDS lifters contaminating the post-cat sensor element. 2004-2010 Ford F-150 5.4L 3V Triton throws P0165 from silicone contamination after a valve-cover gasket replacement with the wrong RTV — Ford specifies only sensor-safe Motorcraft TA-31 for exhaust-adjacent seals. 2008-2013 Mercedes ML550/GL550 4.6L M278 V8 throws P0165 from coolant intrusion through a weeping head gasket fouling the post-cat sensor. Estimated repair: $180 to $600.
